UnitedHealthcare members may access a printed or digital vaccine record

After you’ve been vaccinated, you may be asked to present proof of vaccination to participate in some activities. Your primary proof of vaccination is your CDC COVID-19 Vaccination Card. In addition to this, UnitedHealthcare members have a few other options to use for digital proof of your vaccination:

What is the COVID-19 vaccine pass from UnitedHealthcare?

The UnitedHealthcare COVID-19 vaccine pass uses a QR code to digitally display our records of your vaccination status.1  You can save this pass in your phone for when you need it, which may help eliminate the need to bring your CDC COVID-19 Vaccination Card to events and when you travel. How to create a vaccine pass for a dependent that is 13 years old or older

Dependents 13 years old and older will need to create their own myuhc.com account to be able to generate a vaccine pass. The COVID-19 vaccine pass is not available for self-reported COVID-19 vaccine records.

How to create a vaccine pass for a dependent under 13 years old

Only the digital vaccine record, which shows a member’s vaccinations, is available for this population at this time. Enhancements are underway to offer a vaccine pass, which is a QR code showing vaccination status, to the under 13 population and will be completed in 2022.

  • Your vaccination provider may have provided you a digital record of their patient portal.
  • Your UnitedHealthcare health record or specific COVID-19 vaccine record is available when you sign in to your health plan account at myuhc.com or medicare.uhc.com. If no record shows when you sign in, you may self-report your vaccine.
  • A UnitedHealthcare vaccine pass with QR code may be available to many UnitedHealthcare members when you sign in to your health plan account. This vaccine pass is created using UnitedHealthcare records of vaccination. It is currently not available for self-reported vaccinations.
  • The vaccine record includes the manufacturer and vaccination date, including dates for multiple doses.
  • The information was gathered from UnitedHealthcare records, including claims and information from federal and state agencies, as well as vaccination providers.
  • COVID-19 vaccination records are not currently available for all members due to the many vaccination sites and reported data which may currently be incomplete. 
  • Sign in to your health plan account and go to the vaccine page. The vaccine record link is at the top after you sign in.
  • If your vaccine record is not showing, you may complete a form and upload your CDC Vaccination Card to get a self-reported digital vaccine record.
  • You can use a digital COVID-19 vaccine pass with QR code or print a PDF.
  • The pass can also be used with vaccine verifiers, including CLEAR Health PassCommon Pass Digital health app and New York’s Excelsior Pass.
  • The COVID-19 vaccine pass is not available for all members due to the many vaccination sites and reported data which may currently be incomplete. 
  • The COVID-19 vaccine pass is not available for self-reported COVID-19 vaccine records.
  • Sign in to your health plan account and go to "Vaccine record and resources."
  • If you have a vaccine record on file, choose "Create vaccine pass" and accept the Terms & Conditions to create your vaccine pass. 
  • Activating your vaccine pass is optional, so you can decide when and if you’d like a digital COVID-19 vaccine pass with a QR code. 
  • When you attend an event, present the vaccine pass to scan the QR code. Check with the destination prior to attending to assure the QR code will work.
